Introducing Birthday Recalls for Chronic Disease Management

From 23rd February 2023

We are changing how we invite our patients for their annual review appointments for certain medical conditions such as -

·         Diabetes type1/type2

·         Asthma

·         COPD

·         Hypertension

·         Chronic Heart Disease

·         Hyperthyroid

·         Chronic Kidney Disease

·         Rheumatoid Arthritis

·         Epilepsy

·         Mental health

·         Dementia

You will receive correspondence from Arran Medical Group prior to your birthday month either by letter or SMS with details on how to book your appointment with appropriate clinician.
Please then contact our reception team who will identify the appointments you need to support your care.

This new policy will simplify the system.  It will help you, as a patient, to know when you are due your review, and for the practice staff to identify when a patient’s review is due.

This does mean that you may have slightly longer or slightly less than 12 months between your last review and this year’s review. However, please be assured that you will from then on always have your review in your birth month.

Please note, some reviews occur every 6 months. If this is the case, one review will be during your birthday month and other will be 6 months later.

We will try to ensure that if a patient has multiple conditions, these will all be reviewed in the fewest number of appointments possible.
If you feel you need to be seen before your review please do not hesitate to get in touch with the practice.

As always, for any concerns or problems you have at any time, please call the practice to make an appointment with the most appropriate clinician.
